Each consultation and testing are based on your unique needs. Cost for nutritional supports and shipping are extra.
Comprehensive Consultation is a 2-hour service that includes an one-on-one assessment and testing with informational imprinting, treatment tools and education. $295
Follow up/Recheck Consultation is a 1-hour appointment that includes: one-one assessment and testing with informational imprinting treatment tools and education. $165
Remote Testing is for established clients - includes testing, informational imprinting & treatment tools. $75 - $165
Allergy Testing & ASSERT Treatment is a 1 hour appointment that includes one-on- one assessment and testing with informational imprinting, treatment tools and Erchonia cool laser ASSERT treatment. $200
Healthy 4 Life is not accepted by health insurance or Medicare plans. Healthy 4 Life LLC allows you independence to truly receive a personalized holistic health approach that promotes active participation in creating a optimal healthy state in body, mind & spirit. This aligns with the inner freedom that intuitively yearns to be engaged directly with life itself. Healthy 4 Life shares information for you to decide on what care you receive, rather than allowing an insurance or other plan make decisions for you.
Cancellations and No Shows
While there is understanding that things come up and emergencies happen however, visit slots are limited. Please notify the office if you need to cancel your appointment 2 business days in advance so the appointment can be offered to another person. For no shows without reasonable explanation, Healthy 4 Life LLC reserves the right to charge a minimum of $75 for the unused appointment.
Payment
Healthy 4 Life LLC is a fee for service holistic practice. Payments are made directly at each appointment preferably by cash or check however payment by card with applicable fees are accepted. Healthy 4 Life LLC reserves the right to charge a $35 fee for returned checks.
